Families may still complete the 2025-26 Back-to-School Forms to ensure that your student receives proper services and important communications by email, U.S mail, phone or text.
*Only one parent or guardian in the primary household is expected to complete the form. More resources are available at spps.org/families and click on “Back to School Forms".
